THE EVERYDAY
care designed around the life you already live
What nurse-directed care looks like at home
A registered nurse builds and supervises the care plan, and can delegate tasks most non-medical agencies cannot take on, including simple wound care, ostomy care, diabetic blood sugar checks, tube feeding, and catheter care.
Every plan starts with a free in-home assessment. A Care Navigator visits, listens, and builds the schedule around the routines already in place, rather than asking a family to reshape the week around ours.
