Wayfinder Care Company is committed to protecting the privacy of your health information. This notice explains how we may use and share that information, and the rights you have over it.
Protected health information means information that identifies you and relates to your physical or mental health, the care you receive, or payment for that care. It includes what you tell us during an assessment, what appears in your plan of care, and the notes our Caregivers keep about your visits.
The forms on this site ask for a name, a phone number, an email address and the area you live in, so we can call you back and route your inquiry to the office that serves you. They do not ask for a diagnosis, a medication list or any other health detail, and you should not send those through a web form or a chat window. Bring them to your assessment instead, where they can be recorded properly.
If you do share health information with us through this site, we treat it as protected health information from the moment we receive it, and everything below applies to it.
Anything beyond the above needs your written authorization, and you can withdraw that authorization at any time. We do not sell your health information, and we do not use it for marketing without your written permission.
We are required by law to protect your health information, to give you this notice, and to follow the terms of the notice currently in effect. If your information is breached in a way that puts you at risk, we are required to notify you.
We use outside companies for scheduling, records and communication. Where any of them may handle your health information, we require a Business Associate Agreement obliging them to protect it to the same standard we do.
We may change this notice and apply the change to information we already hold. The current version is always the one on this page, and the effective date is shown below.
To exercise any right above, or to raise a concern, contact our Privacy Officer, Clinton Wood, at contact@wayfindercares.com or call 877.451.5550. You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.
Effective date: 2 October 2026
